Request for change (also known by its acronym, RFC; alternatively known as change request) is a description of a proposed change to modify a document, deliverable, or baseline.

Request for change (RFC). A description of a proposed change used to initiate change control.

According to the PMI Lexicon of Project Management Terms,

Change Request. A formal proposal to modify a document, deliverable, or baseline.
